MetroHealth Project Lead - Pathways HUB - Better Health Partnership in CLEVELAND, Ohio

Location: METROHEALTH MEDICAL CENTER

Biweekly Hours: 80.00

Shift: 8am-430pm

Summary:

In conjunction with the HUB Operations Team and Better Health Partnership leadership, ensures the Better Health Pathways HUB delivers a high-quality experience for clients of the HUB, including at-risk pregnant women, infants, children, and adults with chronic disease and/or behavioral health conditions. Builds trusting, effective relationships with, and supports the community health workers (CHWs) and their supervisors of the Care Coordination agencies (CCA) that partner with the HUB to help them effectively meet the clients’ health, socio-economic, and educational needs. Works closely with the HUB Operations Team and the Pathways HUB Community Advisory Board to identify opportunities and implement strategies for growth and improvement in alignment with HUB operational, quality and financial goals and the needs of the clients served. Seeks opportunities to engage in HUB-related community events and with resource referral partners, as well as in state-wide Pathways HUB advocacy activities. Upholds the mission, vision, values, and customer services standards of The MetroHealth System.

Qualifications:

Required: Bachelor’s Degree in a health-related field. In lieu of degree, any equivalent combination of education, training, and experience in addition to the experience stated below. Four years combined progressively responsible operations and project management experience in a healthcare setting including planning, analysis, and quality improvement. Excellent interpersonal skills required along with experience in working with multi-disciplinary groups. Demonstrated proficiency with Microsoft Office Suite. Excellent communication, presentation and interpersonal skills.

Preferred: Master’s Degree in Public Health, Social Work, Nursing, and/or Health Care Administration. Licensed Social Worker or Registered Nurse. Leadership/ Supervisory experience

Physical Requirements: Ability to communicate in face-to-face, phone, email, and other communications and meetings. Ability to travel to various community settings for meetings and events May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to use computer.
